80% Of Executives Saw AI Incidents Reach The Board, But Only 40% Changed Policy

An IBM study found 80% of executives saw AI incidents reach the board in the past year, yet only 40% updated their AI governance policies.

An IBM study published October 14 surveyed more than 3,200 executives in Europe and the Middle East. Eighty percent reported AI-related incidents reaching board level in the past year, yet only 40% updated their AI governance policies in response. Only 37% say their governance frameworks are built to scale as AI agents gain autonomy. Fifty-six percent proceed with deployment within risk limits while 33% delay.

Norm's Take

The most dangerous AI incident is the one your board already knew about.

Before regulators read it as notice without response, ask for:

  1. Two numbers from your audit committeeAI incidents that reached the board, and how many produced a documented policy change.
  2. Governance that scales with autonomyOnly 37% say their frameworks are built for agents that act on their own.
  3. Owners for the blind spotsName who is accountable for financial risk measurement and supply chain AI risk.
