CNN’s exclusive reporting reveals a US military AI system generated a false intelligence report this spring claiming a Chinese vessel was carrying nuclear weapons components, a hallucination that came close to prompting a military response before it was manually caught. The Department of War has not detailed what process changes or accountability measures followed.

This is the sharpest evidence yet that “human in the loop” governance is theater unless the human has both the time and the authority to override the system under pressure. Any executive citing human oversight as an AI risk control needs to specify who exactly is watching, on what time budget, with what override authority.
